AgeOptions is a not-for-profit organization located in Oak Park, Illinois, that has served the older adults of suburban Cook County and their families as an Area Agency on Aging since 1974. As part of the nationwide network, we are authorized by the federal Older Americans Act and the Illinois Department on Aging as the Planning and Service Area for the 30 townships surrounding Chicago.

Established by the Illinois Violence Prevention Act of 1995, the Illinois Violence Prevention Authority (IVPA) is the first state agency of its kind dedicated to violence prevention in the United States. In creating the IVPA, the Illinois State Legislature recognized the need for a comprehensive, collaborative public health and public safety approach to violence prevention.

Community Memorial Foundation (CMF), established in 1995, is a private independent foundation with a focus on community health improvement. Our geographic target area includes 27 communities in western Cook and southeastern DuPage Counties in Illinois. We define health in the broadest of terms to include physical, mental, social, and environmental well being. Our focus embraces the needs of infants, children, teenagers, and adults as they travel through the path of life. Family health and well being, also broadly defined in order to encompass traditional and non-traditional structures, is a core Community Memorial Foundation focus.
